Select this payment if your child will be attending the BEAM Isalmaic Intensive Summer Program

Age: 4-14-year-olds

Dates: 8 weeks: June 12th- July 31st, Sundays

Time: One-hour slot within the time frame between (11-2 pm) (depends on the child’s level/ age).

Cost: Two installments of $45 or one payment of $85.

Select this payment if your child will be attending the BEAM Arabic Intensive Summer Program

Dates: 6 weeks: June 15th- July 22nd, Mondays, and Wednesdays

Time: One-hour slot within the time frame between (11-2 pm) (depends on the child’s level/ age).

Cost: Tow installment of $75 or one payment of $145 including registration and material fees (which will be available for pick up).

Dates: 6 weeks: June 16th- July 23th, Tuesdays, and Thursdays.

Cost: Tow installment of $75 or one payment of $145 including registration and material fees (which will be available for pick up).

Select this option if your child is doing more than one program so you can get the multi-program discount.

If the child will be registered in (Arabic or Quran) and Islamic Studies, there will be $20 dollars in total discounts, so the total charge will be two installments of $100 or one payment of $ 195 for both programs.

If the child will be registered in the Arabic and Quran program, there will be $30 dollars in total discounts, so the total charge will be two installments of $135 or one payment of $260 for both programs.

If the child will be registered in all three programs (Arabic, Quran, and the Islamic studies program), there will be $50 dollars in total discounts for the three programs, so the total charge will be two installments of $170 or one payment of $ 335 for both programs.
Media Release Form for Minor Children
BEAM (Building & Enriching American Muslims) has my permission to use my or my child’s photograph publically to promote BEAM. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensations shall become payable to me by reason of such use.
Waiver and Release of Liability
I agree that this Waiver and Release of Liability shall apply to each day I am at BEAM’s rented program facility regardless of the date that this form is signed below. I agree I will assume the risk and full responsibility for any and all injuries, losses, or damages, that might occur to my child or other family members while on the premises where BEAM programs are being held or participating in any off-site activities; and to the maximum extent of the law, I agree to waive and release any and all claims, suits, or related causes of action against BEAM, their owners, officers, employees or agents for injury, loss, death, costs, or other damages to me, my heirs or assigns, or third party claims suits or related causes of action asserted against BEAM programs or activities arising from my conduct and/or family’s conduct while participating in BEAM’S programs and activities. I further agree to release, indemnify, defend and hold BEAM harmless from any liability whatsoever for future claims presented by my child for injuries, losses or damages.
Payment and Fees
- Parents or guardians agree and understand that a 30 day written notice of withdrawal must be submitted before the first date of the program that parents or guardians wish to withdraw the child from the program. Notice should be turned in by email to beamofnoor@gmail.com. If notice is received 30 days prior to official withdrawal date, auto payment will stop after 30 days of notice. If no notice is received, auto payment will continue till the end of the registration period. No notice implies that your child will return at some point of the program and your spot is being held for your child and can not be offered to anyone.
